	a) Fully completed Death Claim Form b) Medical report pertaining to the cause of death (where applicable) c) Original Policy Document d) Original Death Certificate e) Certified true copy of Deceased's IC/Birth Certificate f) Certified true copy of Claimant's IC g) Others: Biopsy, MRI, scan and investigation results (where applicable) h) Original post mortem report (for Accidental Death only) i) Police report (for Accidental Death only) j) Newspaper cutting (for Accidental Death only) k) Hospital's itemized bills l) Hospital's original receipts.
